Good script - but a bit confusing.  I am confused as to where the code goes.  I have the first - client side script code in the <head> tags, however what is submit.asp?  I am adding this to our newsletter.  So the newsletter is default.htm in the Newsletter directory.  I assume I should rename to an asp file and add the script in the head.  But then where does the server side script go?  In the same page?  If so, ok great, then is there any code needed for the results page?  A little confused.

Nice script. Quick and easy for the view and the user. But some viewers are getting an error. Any known problems with this code and AOL's browser? Vistitors with an AOL accounts but running IE4/5 on top of AOL have no problems. Those using just AOL to browse are kicked off with a runtime error that point to the following line of code:

var d = w.document

Any ideas?

The survey doesn't run on this site.  I get an error message that says the script is invalid.

I can't get past the following error in submit.asp:

Run time error: object required "request" in line

For Each Item in request.form

What am I doing wrong?

OK, I got rid of the error.

Could you give me some ideas as to how to handle the results of my survey.

I think that this is great but I keep getting this error:

Response object error 'ASP 0156 :
80004005'

Header Error

It would help if there were a "dummies" set of instructions about where to place the asp file, etc.

There should be a troubleshooting section. I can't it to read the cookie. I thought it was the expire date which you set to 1/1/1999. I changed it and now it writes the cookie.

I know it has something to do with the fact that it's installed on a virtual directory.
